A lab-made drug meant to kill drug-resistant germs and block the COVID virus. It works in lab dishes, but no results have been published from trials in people.

Sold for

Brilacidin is promoted as a synthetic mimic of the body's own defensin peptides that could kill drug-resistant bacteria and, during the pandemic, block SARS-CoV-2.

Studies say

Cell-culture studies show it disrupts viral particles and infected cells at low concentrations, but no peer-reviewed human trial has been published for any use (grade C).

Why grade C. The two citable studies are laboratory experiments showing brilacidin inhibits SARS-CoV-2 and other viruses in cell culture. The company has run phase 2 trials in skin infection and COVID-19, but those results exist only as press releases and conference material, not peer-reviewed publications, so the antiviral and antibacterial claims rest on in vitro data (grade C). 12

FDA. Brilacidin is an investigational compound from Innovation Pharmaceuticals and is not approved by the FDA for any use. It has been tested in phase 2 trials for skin infections, oral mucositis, ulcerative proctitis and COVID-19, none of which has led to a marketing application. 12

What this means for you

What people take it for
Fighting resistant germs and COVID.
Can a US doctor prescribe it?
Not yet; it is still in trials.
Properly tested in people?
Not properly: animal or lab studies, or small uncontrolled human reports. The company reports trials only in press releases, and a 120-patient COVID trial showed no gain over a dummy treatment.

What it is

Brilacidin is not strictly a peptide. It is a small lab-made molecule (an arylamide). It was designed to copy the shape and charge pattern of defensins. Defensins are short antimicrobial peptides. Immune cells and the body's surface cells release them as a first line of defence. The goal was to keep the way defensins break open germ membranes. The drug would also be stable and easy to make, like a usual drug. 12

PolyMedix developed it in the 2000s. Innovation Pharmaceuticals (formerly Cellceutix) bought it in 2013. It has been made as an infusion into a vein for skin infections. It has also been made as a mouth rinse for mouth sores from chemotherapy, a rectal form for ulcerative proctitis and an infusion for COVID-19. 1

How it works

Defensins and their copies carry a positive charge. They are also amphipathic, which means one face loves water and the other loves fat. They stick to germ membranes and virus coats, which carry a negative charge. They push in and break the membrane. The germ then leaks and dies. This is a physical attack on the membrane. It does not block one enzyme. So bacteria find it harder to become resistant. 12

Against SARS-CoV-2 in cell culture, brilacidin blocked infection, and more drug blocked more infection. The effect was strongest when the virus met the drug before it reached cells. That fits damage to the virus coat. Later lab work found activity against three seasonal human coronaviruses (OC43, 229E and NL63). It also found a second mechanism, binding to heparan sulfate on the surface of host cells. The company has also described anti-inflammatory effects through blocking phosphodiesterase enzymes. 12

What human research shows

No peer-reviewed human trial of brilacidin has been published. The company has made three claims in press releases and regulatory filings. First, a phase 2b trial in acute bacterial skin and skin structure infections found one dose by vein worked about as well as seven days of daptomycin. Second, a phase 2 proof-of-concept trial in ulcerative proctitis was positive. Third, a randomized, placebo-controlled phase 2 trial in 120 hospitalised COVID-19 patients reported topline results in November 2021. Brilacidin did not differ from placebo on the main outcome, time to sustained recovery through day 29. None of these datasets has appeared in a journal where others can check the design and results. 123

Safety

In vitro studies report low toxicity to human cells at doses that blocked virus. Human safety data from the phase 2 programmes have not been published in a journal. The company's COVID-19 topline release described side effects at similar rates in the brilacidin and placebo arms. Earlier defensin-mimetic compounds in this class raised worries about blood pressure effects and numb mouths during infusion. Public data cannot show whether brilacidin shares those effects. 123

Questions for your clinician

  • Has brilacidin been tested in a published human trial for the condition I have?
  • What is known about its safety in people?
  • Are there approved treatments I should consider first?

Questions people ask

Is brilacidin FDA-approved?

No. It has been through phase 2 trials in several conditions. But no one has filed for approval, and no trial results have been published in a peer-reviewed journal. 12

Is brilacidin a peptide?

Not chemically. It is a small lab-made molecule built to imitate defensin peptides. It has the same positive charge and two-faced shape that let those peptides punch holes in germ membranes. People group it with peptides because of what it copies. 1

Did brilacidin work against COVID-19?

In cell culture, yes, at doses that did not harm human cells. A placebo-controlled phase 2 trial in 120 hospitalised patients missed its main goal, according to the company's November 2021 topline release. The full results have not been published in a journal. 13

Why is it interesting for antibiotic resistance?

It attacks bacterial membranes physically and does not block a single enzyme. So bacteria have a harder time evolving resistance. Whether that holds up in patients is unknown, as the skin-infection trial data remain unpublished. 1
