An ingredient in skin creams sold to protect against sun damage and aging. It has only been tested on pieces of skin in a lab, not on people's faces.
Sold for
Carnosine is marketed in serums and creams as an antioxidant and anti-glycation ingredient that protects skin from sun damage and ageing.
Studies say
Lab models of human skin show carnosine lowers oxidative stress markers after UV exposure, but nobody has shown a visible benefit in a controlled trial on people, so the evidence is C.
Why grade C. For the skin anti-ageing use, the available data are an ex vivo study of human skin explants exposed to simulated sunlight and a 3D fibroblast spheroid model under UVA. No randomized trial on living volunteers is in the citations, so the grade is C. 12 A 2026 randomized trial of oral carnosine in 299 adults measured exercise performance, not skin, and found mostly no effect, so it does not change the skin grade. 3
FDA. Carnosine in skincare is a cosmetic ingredient and sits outside FDA drug approval; no carnosine product is approved by the FDA as a drug for any use. 1

What this means for you
- What people take it for
- Protecting skin from sun damage and aging.
- Can a US doctor prescribe it?
- No prescription; it is a cosmetic ingredient.
- Properly tested in people?
- Not properly: animal or lab studies, or small uncontrolled human reports. A 299-adult trial of carnosine pills looked at exercise, not skin, and mostly found no effect.

What it is
Carnosine is a dipeptide, two amino acids (beta-alanine and histidine) joined together, that occurs naturally at high concentration in human muscle and brain. The skincare industry uses the synthetic form, usually labelled L-carnosine, as an antioxidant additive. 1
In cosmetics it is an ingredient, not a drug, so it does not go through FDA approval. The regulatory bar is that the finished product is safe and not marketed with drug claims. 1
How it works
Carnosine can scavenge reactive oxygen species and bind reactive aldehydes, which are the molecules that attach sugars to proteins and form advanced glycation end-products (AGEs), the cross-links blamed for stiff, aged collagen. In a 3D spheroid model of human skin fibroblasts exposed to UVA, carnosine protected cells and switched on the Nrf2 pathway, a cellular stress-response program that raises levels of detoxifying enzymes. 21
What human research shows
No controlled trial of carnosine on skin in living people appears in the citations. The closest human data come from skin biopsies kept alive in culture: an Italian group exposed human skin explants to solar-simulated radiation and applied L-carnosine topically at two concentrations. Carnosine reduced reactive oxygen species and AGE formation and blunted the changes in a panel of marker genes. Explants cannot report redness, wrinkles or pigmentation over time, so these are mechanistic results, not clinical ones. 1
The one randomized trial of carnosine in living people tested the oral supplement for exercise performance, not the skin. O'Toole and colleagues randomized 299 adults to oral carnosine or placebo for 12 weeks and measured grip strength, calf raises to exhaustion, a two-minute step test and gait speed. Most comparisons showed no difference. The exceptions were more calf raises at six weeks in participants under 40 and more steps at 12 weeks in men over 40, both subgroup results. The trial says nothing about carnosine applied to skin. 3
| Study | Model | Finding |
|---|---|---|
| Human skin explants under simulated sun | Ex vivo human skin biopsies, topical L-carnosine at two concentrations | Lower reactive oxygen species and AGE formation after solar-simulated radiation. 1 |
| UVA-exposed fibroblast spheroids | Cells, 3D human skin fibroblast spheroids | Carnosine protected against UVA damage and activated the Nrf2 antioxidant response. 2 |
| O'Toole 2026, oral carnosine and endurance | Humans, n=299, randomized placebo-controlled, 12 weeks oral | No difference from placebo on most performance measures; small gains in two age and sex subgroups. 3 |
Safety
Topical carnosine has not been linked to specific adverse effects in the cited work, and it is a compound the body already makes. What is unknown is whether the concentrations used in skin explants match what reaches the dermis from a consumer product, and whether any effect persists with daily use. 12
Questions for your clinician
- Is there a controlled trial of carnosine on skin in living people?
- Do the concentrations in skin products match those used in lab studies?
- Would a proven ingredient fit my skin goal better?
Questions people ask
Is carnosine FDA-approved?
Not as a drug. In skincare it is a cosmetic ingredient, which does not require FDA approval, and no carnosine drug product exists in the United States. 1
Does carnosine reduce wrinkles?
There is no controlled trial in people showing that. The published work uses skin explants and cell models, which measure oxidative-stress and glycation markers, not wrinkles. 12
What does anti-glycation mean?
Glycation is the reaction of sugars with proteins such as collagen, producing cross-links called AGEs that make tissue stiffer. Carnosine can bind the reactive intermediates of that process, and in a skin-explant model it reduced AGE formation after UV exposure. 1

Updates
- 2026-10-27 added O'Toole 2026 RCT of oral carnosine for endurance (n=299, mostly null; PMID 42308284) to human findings and key studies; grade stays C for the skin use.
