A drug given during dialysis to lower a gland hormone that rises in kidney disease. Three controlled trials show it lowers that hormone, but none tested whether it prevents fractures or deaths.
Sold for
Etelcalcetide is an intravenous calcimimetic that lets dialysis units control parathyroid hormone without relying on patients to take pills.
Studies say
Three randomized trials show it lowers parathyroid hormone more than placebo and more than oral cinacalcet, an A for the approved biochemical indication.
Why grade A. Approval rests on two placebo-controlled phase 3 trials (1,023 patients combined) and a head-to-head trial against cinacalcet in 683 patients, all randomized and double-blind, all showing larger reductions in parathyroid hormone. The endpoint is a laboratory marker; no trial has shown that lowering PTH with etelcalcetide reduces fractures or deaths. 12
FDA. Approved. Etelcalcetide was approved by the FDA in February 2017 as Parsabiv (Amgen) for secondary hyperparathyroidism in adults with chronic kidney disease on hemodialysis. It is given intravenously through the dialysis circuit at the end of a session. 2

What this means for you
- What people take it for
- High parathyroid hormone in kidney disease
- Can a US doctor prescribe it?
- Yes, FDA approved for high parathyroid hormone in adults on dialysis.
- Properly tested in people?
- Yes, in large controlled trials of 1,023 and 683 people. They measured a blood test, not fractures or deaths.

What it is
Etelcalcetide is a synthetic eight-amino-acid peptide. Seven of its amino acids are D-isomers (mirror images of the natural forms), which protects it from enzymatic breakdown, and it is linked by a disulfide bond to a single L-cysteine. It is the second calcimimetic drug after cinacalcet and the first that can be given intravenously. 12
In advanced kidney disease the parathyroid glands enlarge and overproduce parathyroid hormone (PTH) in response to low calcium, high phosphate and low vitamin D. The resulting condition, secondary hyperparathyroidism, weakens bone and contributes to blood-vessel calcification. Parsabiv was approved in 2017 for hemodialysis patients with this condition. 2
How it works
The parathyroid gland senses blood calcium through the calcium-sensing receptor. Etelcalcetide binds that receptor at a site distinct from calcium itself and makes it more sensitive, so the gland behaves as if calcium were higher than it is and reduces PTH secretion. Because the drug is given at the end of dialysis and cleared largely by dialysis itself, its blood levels stay steady between sessions. Lowering PTH this way also lowers blood calcium, which is the mechanism behind its main side effect. 12
What human research shows
In two identical placebo-controlled trials pooled in the label, 1,023 hemodialysis patients with secondary hyperparathyroidism were randomized to etelcalcetide or placebo for 26 weeks. In each trial 77 to 79% of etelcalcetide patients achieved a PTH reduction of more than 30%, compared with 11% on placebo. Corrected calcium also fell, and phosphate fell modestly (significantly in one of the two trials). 2
The head-to-head trial randomized 683 patients to intravenous etelcalcetide plus oral placebo or oral cinacalcet plus intravenous placebo for 26 weeks. A PTH reduction of more than 30% was achieved by 68.2% of etelcalcetide patients and 57.7% of cinacalcet patients, meeting the noninferiority goal and then superiority. A reduction of more than 50% was seen in 52.4% versus 40.2%. Both drugs caused nausea and vomiting at similar rates. 1
What none of these trials measured is whether patients fare better. Cinacalcet's own outcomes trial (EVOLVE, not cited here) failed to show a reduction in death or cardiovascular events, and no outcomes trial of etelcalcetide has been run. 12
| Study | Model | Finding |
|---|---|---|
| Block 2017 head-to-head trial | Humans, n=683, randomized double-blind double-dummy, etelcalcetide vs cinacalcet, 26 weeks | A greater than 30% PTH reduction in 68.2% of etelcalcetide patients versus 57.7% on cinacalcet. 1 |
| Pooled placebo-controlled trials (label) | Humans, n=1,023, two randomized double-blind placebo-controlled trials, 26 weeks | In each trial 77 to 79% of etelcalcetide patients achieved a greater than 30% PTH reduction versus 11% on placebo. 2 |
Safety
Low blood calcium is the characteristic adverse effect: in the head-to-head trial, laboratory hypocalcemia was recorded in a majority of patients in both arms and symptomatic hypocalcemia (muscle cramps, paresthesia, seizures in rare cases) occurred in about 5% on etelcalcetide. Low calcium can prolong the QT interval and worsen heart failure, so the label requires calcium monitoring and advises caution in patients with seizure disorders or heart failure. Nausea, vomiting and diarrhea were the other common events. Because the drug is cleared by dialysis, it is not suitable for patients not on hemodialysis. 12
Questions for your clinician
- Does lowering PTH with etelcalcetide reduce my fracture or death risk?
- How is my calcium checked after each dialysis session?
- How does it compare with taking cinacalcet pills?
Questions people ask
Is etelcalcetide FDA-approved?
Yes. Parsabiv was approved in 2017 for secondary hyperparathyroidism in adults with chronic kidney disease on hemodialysis. 2
How does etelcalcetide differ from cinacalcet?
Both sensitize the calcium-sensing receptor, but cinacalcet is an oral small molecule taken by the patient and etelcalcetide is a peptide the dialysis staff give intravenously. In the head-to-head trial etelcalcetide lowered PTH in more patients, with similar rates of nausea. 1
Does lowering PTH with etelcalcetide prevent fractures or heart problems?
That has not been shown. The trials measured PTH and other blood markers, not clinical outcomes. The oral calcimimetic cinacalcet failed to show a mortality benefit in its outcomes trial, and etelcalcetide has not had one. 12
What is the main side effect?
Low blood calcium, which can be asymptomatic or cause cramps, tingling and in rare cases seizures or heart rhythm changes. Calcium is monitored during treatment. 2
