It is prescribed by anti-ageing clinics to raise growth hormone for fat loss, muscle and sleep. It does raise the hormone, but no trial has tested it for those goals in healthy adults.
Sold for
Sermorelin is sold by anti-aging and men's health clinics as a gentler way to restore youthful growth hormone levels, promising fat loss, muscle, deeper sleep and recovery.
Studies say
Sermorelin does raise growth hormone and IGF-1, and small physiology studies show GHRH increases slow-wave sleep, but the fat loss and muscle data come from trials of tesamorelin in HIV patients, and no trial has tested sermorelin for anti-aging in healthy adults.
Why grade C. For the use sermorelin is sold for today, raising growth hormone in healthy middle-aged adults to lose fat, build muscle and sleep better, there is no randomized trial of sermorelin. The controlled human data are physiology experiments in 7 to 13 people using GHRH, a retrospective series of 14 men on a combination regimen, and randomized trials of a different GHRH analog, tesamorelin, in HIV-associated belly fat (806 patients pooled) and in 60 obese adults. Class evidence from another drug in another population does not lift sermorelin itself above C. 1234567812
FDA. Sermorelin was FDA-approved as Geref for diagnosing growth hormone deficiency and for treating growth failure in children with idiopathic growth hormone deficiency. EMD Serono discontinued Geref in 2008, and in 2013 the FDA determined it was not withdrawn for reasons of safety or effectiveness, which allows the agency to approve generic versions, though none has been approved. There is no FDA-approved sermorelin product on the market; what clinics prescribe is compounded, and the anti-aging and body composition uses it is prescribed for were never approved.

What it is
Sermorelin is a lab-made copy of the first 29 amino acids of growth hormone-releasing hormone (GHRH). GHRH is the hormone from the hypothalamus that tells the pituitary to release growth hormone. Those 29 amino acids are the fully active part of the 44-amino-acid hormone. It was approved in the 1990s as Geref. It was an injection to test pituitary function and to treat children whose growth hormone release was too low. The maker stopped making Geref in 2008. No company has brought a sermorelin product back to market. 10
The FDA decided in 2013 that Geref was not pulled for safety or effectiveness reasons. That lets the agency approve generic versions, but none has been approved. What clinics prescribe is compounded. It has become a staple of telehealth and anti-aging clinics. They often pair it with growth hormone secretagogues such as ipamorelin or GHRP-2. A 2026 review of performance peptides puts sermorelin in the tier of compounds with regulatory-grade data for a different approved use. The online protocols have never been tested. 110
How it works
GHRH binds receptors on somatotroph cells in the pituitary and triggers pulses of growth hormone release. Growth hormone then acts on the liver and other tissues to make IGF-1. IGF-1 carries most of its effects on growth and metabolism. Sermorelin works through the pituitary, so the body's feedback loops stay in place. Somatostatin is the hormone that shuts off growth hormone release. It still acts when levels are high. That is the basis of the claim that sermorelin cannot push growth hormone as high as injected growth hormone can. Growth hormone release declines steadily from early adulthood. Some call this the somatopause. Part of the decline reflects lower GHRH output from the hypothalamus. So GHRH replacement is a logical idea. In a 1989 experiment, repeated GHRH doses in healthy men aged 65 to 78 restored the blunted growth hormone response to a GHRH test. 151314
What human research shows
Sermorelin itself has almost no modern trial data. A 2017 retrospective chart review looked at men on testosterone therapy. They had been prescribed sermorelin together with GHRP-2 and GHRP-6. In the 14 men who met compliance criteria, IGF-1 rose from a mean of about 160 to about 240 ng/mL over roughly four months. There was no control group. The plan combined three drugs. No one measured body composition. 6
The body composition claims come from tesamorelin, a stabilized form of full-length GHRH. It was tested in people with HIV and belly fat linked to antiretroviral drugs. In a 412-patient randomized trial, tesamorelin cut visceral fat by 15 percent over 26 weeks. Placebo patients gained 5 percent. Triglycerides also improved. A pooled analysis of two phase 3 trials (806 patients) confirmed the visceral fat drop. It held at 52 weeks in those who stayed on the drug. Subcutaneous fat did not change. A secondary analysis found small gains in trunk muscle density and area. In 60 obese adults without HIV who had low growth hormone release, 12 months of tesamorelin reduced visceral fat and carotid artery wall thickness. It did not worsen glucose. A 2026 meta-analysis of five tesamorelin trials in HIV confirmed the effects on visceral fat, liver fat and lean mass. No one has tested whether compounded sermorelin at clinic doses does any of this. 2341112
The sleep claims rest on small physiology studies. In them, GHRH was given into a vein overnight. In 7 young men, four pulses of GHRH raised slow-wave sleep from 14 to 20 percent of the night. They also blunted overnight cortisol. In another study, a GHRH bolus during the third REM period produced a large rise in slow-wave sleep. In 13 healthy adults in their late 60s, GHRH reduced night-time awakenings. It had much less effect on slow-wave sleep than in the young, and cortisol did not change. These were single-night lab experiments. They were not trials of nightly injections. 789
| Study | Model | Finding |
|---|---|---|
| Tesamorelin in HIV abdominal fat, phase 3 | Humans, n=412 with HIV, randomized, double-blind, placebo-controlled, 26 weeks | Visceral fat fell 15 percent versus a 5 percent rise on placebo, with improved triglycerides and an 81 percent rise in IGF-1. 2 |
| Pooled tesamorelin phase 3 analysis | Humans, n=806 with HIV, two randomized trials plus 26-week extension | Visceral fat reduction maintained at 52 weeks in those continuing treatment; no change in subcutaneous fat. 3 |
| Tesamorelin in obese adults with low GH | Humans, n=60, randomized, double-blind, placebo-controlled, 12 months | Reduced visceral fat, carotid wall thickness, C-reactive protein and triglycerides without affecting glucose. 4 |
| GHRH and sleep EEG in young men | Humans, n=7, placebo-controlled crossover, single night | Slow-wave sleep rose from 14 to 20 percent of the night and overnight cortisol fell. 7 |

Safety
Sermorelin has a long record of single-dose use for diagnosis. The worry with adult anti-aging use is the hormone it raises. Too much growth hormone and IGF-1 are tied to insulin resistance, fluid retention, joint pain and carpal tunnel syndrome. IGF-1 also promotes cell growth. So a theoretical cancer risk is raised whenever this axis is stimulated for a long time. The 2026 review of performance peptides lists the adverse effects clinicians see in users of this class. They are endocrine disturbances, dysglycemia, fluid retention, musculoskeletal pain and injection site reactions. It also notes the added doubt of compounded or unregulated products of unknown makeup. No long-term safety study of sermorelin in healthy adults exists. 112
Questions for your clinician
- Is there any controlled trial of sermorelin for the goal I have in mind?
- Could my symptoms come from a treatable cause other than low growth hormone?
- What do raised IGF-1 levels mean for my blood sugar and cancer screening?
Questions people ask
How is sermorelin different from human growth hormone?
Growth hormone injections supply the hormone directly. They can push levels as high as the dose allows. Sermorelin asks the pituitary to release its own growth hormone. So the body's feedback through somatostatin still limits the peak. That is the argument that it is gentler. It is also why it cannot work in someone whose pituitary cannot respond. 15
What does the GHRH research show about visceral fat?
Randomized trials of tesamorelin, a related GHRH analog, in people with HIV showed about a 15 percent drop in visceral fat over 26 weeks. It held to 52 weeks, with no loss of subcutaneous fat. A 60-person trial in obese adults without HIV showed a similar effect. No trial has tested sermorelin for fat loss in anyone. 234
Can sermorelin change how you sleep?
GHRH given into a vein overnight raised slow-wave sleep in small studies of young men. It reduced awakenings in older adults, with a smaller effect in the elderly. Those were one-night lab experiments. No one has studied whether nightly sermorelin injections improve sleep in practice. 789
What is sermorelin's approval history with the FDA?
As Geref, it was used to diagnose and treat growth hormone deficiency in children. EMD Serono discontinued Geref in 2008. In 2013 the FDA ruled that it was not withdrawn for safety or effectiveness. That allows the agency to approve generic versions, though none has been approved. No approved product exists now. Clinics prescribe compounded sermorelin for uses that were never approved. 10
What is the somatopause?
It is the gradual fall in growth hormone release that starts in early adulthood. Research in older men suggests that part of it comes from the hypothalamus releasing less GHRH. In one small 1989 study, repeated GHRH doses restored the pituitary's response. No one has settled whether reversing the decline helps or harms healthy older adults. 51314
