A self-injected shot for myasthenia gravis, a muscle weakness disease. A trial of 174 patients showed it improved daily symptoms, with a risk of serious infection.

Sold for

Zilucoplan is the first self-injected complement inhibitor for myasthenia gravis, a self-injected shot that replaces hospital infusions.

Studies say

A 174-patient randomized trial showed clinically meaningful improvement in daily-living symptoms over placebo at 12 weeks, with the class risk of meningococcal infection, which supports an A.

Why grade A. Approval rests on RAISE, a 174-patient randomized placebo-controlled phase 3 trial that met its primary endpoint on the patient-reported MG-ADL scale at 12 weeks, plus a phase 2 trial. The approved use matches the trial population, so A; durability beyond 12 weeks comes from open-label extension data. 12

FDA. Zilucoplan is approved by the FDA as Zilbrysq (October 2023) for generalized myasthenia gravis in adults who are anti-acetylcholine receptor antibody positive. It carries a boxed warning for serious meningococcal infections, requires meningococcal vaccination, and is available only through a restricted REMS program. 2

What it is

Zilucoplan is a 15-amino-acid macrocyclic peptide, meaning its chain is closed into a ring, with a fatty acid tail that binds albumin so it lasts long enough for injection under the skin. It was discovered by Ra Pharmaceuticals using a peptide-screening technology and developed by UCB. 12

Generalized myasthenia gravis is an autoimmune disease in which antibodies, most often against the acetylcholine receptor at the junction between nerve and muscle, cause fluctuating weakness of the eyes, face, throat, limbs and breathing muscles. Before complement inhibitors, treatment relied on pyridostigmine, steroids, immunosuppressants and intravenous immunoglobulin or plasma exchange for crises. 1

How it works

Antibodies bound to the acetylcholine receptor recruit the complement system, a cascade of blood proteins that punches holes in the muscle membrane at the nerve junction and destroys receptors. Zilucoplan binds complement protein C5 and stops it from being cleaved into C5a and C5b, which blocks formation of the membrane-attack complex. The same step is blocked by the antibody drugs eculizumab and ravulizumab; zilucoplan does it with a small peptide that patients can inject themselves, where the antibodies are infused. Because C5 is also needed to kill Neisseria bacteria, blocking it raises the risk of meningococcal disease. 12

What human research shows

RAISE, published in Lancet Neurology in 2023, randomized 174 adults with acetylcholine receptor antibody-positive generalized myasthenia gravis at 75 sites in Europe, Japan and North America to zilucoplan or placebo for 12 weeks. The Myasthenia Gravis Activities of Daily Living score, an 8-item patient-reported scale, improved by 4.39 points on zilucoplan versus 2.30 on placebo, a difference of 2.09 points. The clinician-scored Quantitative Myasthenia Gravis scale improved by 6.19 versus 3.25 points. Improvements were seen by the first week. 1

Participants from RAISE could continue into open-label extension studies. The label reports safety data from 213 patients in those studies, including pancreatic events in seven (3.3 percent). 2

Safety

In RAISE, injection-site bruising was the most common adverse event (16 percent on zilucoplan versus 9 percent on placebo), followed by headache and diarrhea. Serious adverse events and serious infections were similar in the two groups. One patient died in each group (COVID-19 on zilucoplan, cerebral haemorrhage on placebo), and neither death was judged related to the drug. No meningococcal infections occurred in the trial. 1

The boxed warning reflects the mechanism: complement C5 blockade leaves patients vulnerable to life-threatening meningococcal infection. Patients must complete or update meningococcal vaccination at least two weeks before the first dose or receive antibiotics until they have, and the drug is dispensed only through the Zilbrysq REMS. Pancreatitis and elevated pancreatic enzymes were observed in the development program and are listed warnings. 2

Questions for your clinician

  • Which vaccines are needed before starting zilucoplan?
  • How does it compare with infusion complement blockers?
  • How long do the benefits last beyond 12 weeks?

Questions people ask

Is zilucoplan FDA-approved?

Yes. Zilbrysq was approved in October 2023 for adults with generalized myasthenia gravis who test positive for acetylcholine receptor antibodies. It is the first complement inhibitor for the disease that patients inject themselves. 2

How does it compare with eculizumab?

Both block complement C5. Eculizumab and ravulizumab are antibodies given by intravenous infusion; zilucoplan is a small peptide given as a subcutaneous injection. No head-to-head trial has been published, so relative efficacy is not established. 12

How much did patients improve in RAISE?

The patient-reported MG-ADL score improved by about 4.4 points on zilucoplan versus 2.3 on placebo at 12 weeks, a difference of roughly 2 points on an 8-item scale; a 2-point change is the usual threshold for clinical meaningfulness. Improvement began within the first week. 1

Why does it require vaccination?

Blocking complement C5 disables the body's main defense against Neisseria meningitidis, the bacterium that causes meningococcal meningitis and sepsis. The boxed warning requires vaccination before starting and the drug is only available through a risk-management program. 2
