Exenatide

Does Exenatide work?

Yes, in large controlled trials, including one of 14,752 patients.

AStrong human evidenceFDA approvedas of 2026-10-26

The diabetes indication rests on a long program of randomized trials summarized in the label and on EXSCEL, a placebo-controlled cardiovascular safety trial of 14,752 patients that showed no increase in cardiovascular events over a median 3.2 years. 18 Weight loss with exenatide averages a few kilograms, well below the 15% and 22.5% seen with semaglutide and tirzepatide in their obesity trials, and it has no obesity indication. 56

The label sums up the trials that led to approval. Patients added weekly exenatide to metformin, a sulfonylurea or both. HbA1c fell by roughly 1.3 to 1.9 percentage points over 24 to 30 weeks. Patients lost 2 to 3 kg. In head-to-head trials against liraglutide and insulin glargine, exenatide did worse or about the same on HbA1c. 1

EXSCEL randomized 14,752 patients with type 2 diabetes. Most had cardiovascular disease. They got weekly exenatide or placebo. Researchers followed them for a median of 3.2 years. Major cardiovascular events occurred in 11.4% of exenatide patients and 12.2% of placebo patients. The hazard ratio was 0.91. That met the safety goal of noninferiority. It narrowly missed statistical superiority. In a secondary analysis, deaths from any cause were lower on exenatide. 8
