Is Desmopressin safe?
Boxed warning for low sodium
The one serious risk is hyponatremia, or low blood sodium. It happens when desmopressin stops the kidneys from passing water and the person keeps drinking. Severe cases cause seizures, coma and death. Nocdurna carries a boxed warning for it, as Noctiva did. The label requires a sodium check before starting and within the first week. It bars use in people with kidney impairment, heart failure or uncontrolled hypertension. It also bars use with loop diuretics and with systemic or inhaled glucocorticoids. 31
The same rule applies to the older approved uses. Fluid intake must be limited in the hours around a dose. This matters most for children treated for bedwetting and for elderly patients. Nasal forms can cause congestion and nosebleeds. The 2020 Stimate recall followed a packaging seal problem. The seal let the solution evaporate. Patients could then get a higher concentration of desmopressin than the label stated. Headache, nausea and facial flushing are the common minor effects. 24
