Gut effects dominate. They include nausea, diarrhea, vomiting and belly pain, and they are worst in the first weeks. The label has a boxed warning about thyroid C-cell tumors seen in rodents. People with a personal or family history of medullary thyroid carcinoma or MEN 2 must not take it. Other warnings cover pancreatitis, gallbladder disease, acute kidney injury from dehydration and diabetic retinopathy complications. Hypoglycemia is also a risk when it is combined with insulin or sulfonylureas. 1

In REWIND, serious adverse events were no more frequent on dulaglutide than on placebo over five years. This is the longest controlled safety follow-up for the drug. 3

Nausea and other gut effects are common. The serious warnings are pancreatitis, gallbladder disease, kidney injury from dehydration and worse diabetic retinopathy. The boxed warning comes from a thyroid tumor signal in rodents. 1
