The Russian studies describe semax as well tolerated, including in older patients. Side effects were few, mostly nasal irritation. FDA staff reviewed it for compounding in 2026. They concluded the published data were too thin to establish safety. They noted there were no controlled trials, pharmacokinetic data or long-term follow-up. Semax is derived from ACTH. So effects on the stress hormone system and on mood with sustained use are theoretical concerns. They have not been studied. Products sold online are not regulated. 2712

The October 2026 HHS policy does not add safety data. HHS said the nasal human record covers several hundred people with no reported adverse outcome and that endotoxin was the FDA's main concern. Its own text says compounded drugs are not FDA-approved and the FDA does not review their safety before sale. News reports say FDA scientists opposed the policy. 1416
