Does Ga-68 dotatate work?
Partly: accuracy studies, such as one of 78 patients, rather than large trials.
As a diagnostic, Ga-68 dotatate was approved on accuracy studies, not randomized trials. The study cited here, 78 patients with known or suspected neuroendocrine tumors imaged with both Ga-68 dotatate PET and the older In-111 pentetreotide scan, found the PET scan more accurate and changed management in 36% of patients. 1 Multiple consistent studies exist and the scan is now standard practice, but the evidence type keeps it at B under our definitions. 12
A Vanderbilt study supported approval. It enrolled 97 consecutive patients with known or suspected neuroendocrine tumors. Of these, 78 had both Ga-68 dotatate PET/CT and In-111 pentetreotide imaging. Ga-68 dotatate was equal or better than the older scan in all 78 patients. It correctly picked out 3 patients for peptide receptor radiotherapy. The older scan had misclassified them. It changed the treatment plan for 36% of participants. Radiation exposure was lower, and the scan took one visit instead of two days. 1
Based on this and similar studies, the label says the scan was tested in patients with known or suspected disease. It warns that uptake is not specific to neuroendocrine tumors. Other tumors can have somatostatin receptors. So can inflammatory conditions. Both can produce false positives. 2
