Lutetium Lu-177 dotatate

Does Lutetium Lu-177 dotatate work?

Yes, in a controlled trial of 229 patients.

AStrong human evidenceFDA approvedas of 2026-10-26

Approval rests on NETTER-1, a randomized open-label phase 3 trial in 229 patients with progressive midgut neuroendocrine tumors, which showed a large improvement in progression-free survival over high-dose octreotide, supported by a Dutch single-center series of more than 1,200 patients. The remaining gap is overall survival, where the final NETTER-1 analysis showed no statistically significant difference. 12

NETTER-1 randomized 229 patients with advanced midgut neuroendocrine tumors. Their tumors had grown on standard-dose octreotide. Patients got lutetium Lu-177 dotatate plus octreotide, or high-dose octreotide alone. At the main analysis, estimated progression-free survival at 20 months was 65.2 percent with the radioligand. It was 10.8 percent in the control arm. That is a 79 percent cut in the risk of progression or death. Response rates were 18 percent against 3 percent. 1

The label also sums up a Dutch series of 1,214 patients treated at Erasmus MC. All got the drug. It gave safety and response data across GEP-NET types. The 2024 extension to patients aged 12 and older did not rest on a separate trial of whether the drug works. The label extrapolates from NETTER-1 in adults. It adds safety, drug-level and radiation-dose data from a few adolescents. Four of them had GEP-NETs. 2

Evidence checklist

  • Randomised controlled trial NETTER-1, open-label vs high-dose octreotide
  • Human study over 100 people NETTER-1, 229; Dutch series 1,214
  • More than one research group Pivotal trial plus one single-centre series
  • Clinical outcomes, not surrogate markers Progression-free survival; no clear survival gain
  • Human safety data beyond case reports Marrow, kidney and leukemia risks documented
