Pemvidutide

Does Pemvidutide work?

Partly: some human trials, with gaps: a 212-person trial lasting 24 weeks so far.

Two randomized, double-blind, placebo-controlled trials have been published: the IMPACT phase 2b trial in 212 patients with biopsy-confirmed MASH and a 64-patient fatty liver trial with 24 weeks of treatment. IMPACT met its MASH-resolution endpoint but not its co-primary fibrosis endpoint at 24 weeks; the smaller trial met its liver-fat endpoint. A phase 3 MASH trial (PERFORMA) began in July 2026. 4 What is missing is phase 3 results, long-term safety data, and any peer-reviewed publication of the obesity trial. 23

IMPACT was published in the Lancet in late 2025. It randomized 212 adults with biopsy-confirmed MASH and moderate to advanced fibrosis. They got one of two doses of weekly pemvidutide or placebo. Doses did not rise gradually, as they do for most GLP-1 drugs. At 24 weeks, MASH resolved without worse fibrosis in 58 percent and 52 percent of patients at the two doses, versus 20 percent on placebo. The second main outcome was fibrosis improvement without worse MASH. It was not met: 33 and 36 percent versus 28 percent on placebo. These differences were not statistically significant. Liver fat on imaging fell sharply, and body weight fell. The trial continues to 48 weeks. 2

The earlier program tested pemvidutide in people with fatty liver disease, without biopsy. A 12-week extension was published in JHEP Reports. In it, 64 participants had finished a 12-week placebo-controlled trial. They kept their assigned treatment, for 24 weeks in total. Relative liver fat fell by about 56 to 76 percent across the three doses, against 14 percent on placebo. More than half of patients at the middle dose reached normal liver fat. Body weight fell by 6.2 percent over 24 weeks. 3
