Daptomycin

Does Daptomycin work?

Yes, in large controlled trials, including one of 246 patients.

AStrong human evidenceFDA approvedas of 2026-10-27

Approval rested on randomized trials against standard antibiotics. The bacteremia and endocarditis trial in the citations randomized 246 patients and found daptomycin non-inferior to standard therapy, with success in 44.2% versus 41.7% at 42 days after treatment, a low absolute rate that reflects how hard these infections are to cure. 12

Fowler and colleagues randomized 246 patients with S. aureus bacteremia, with or without endocarditis. One group got daptomycin. The other got standard therapy, either an anti-staphylococcal penicillin or vancomycin, each with a first low dose of gentamicin. The main measure was whether treatment worked 42 days after it ended. Success was 44.2% with daptomycin (53 of 120) and 41.7% with standard therapy (48 of 115). The gap was 2.4 percentage points, and the confidence interval met the non-inferiority margin. This trial is why bacteremia and endocarditis were added as approved uses. 12

Serious kidney problems occurred in 11.0% of daptomycin patients and 26.3% on standard therapy. Most of that gap was put down to the gentamicin and vancomycin in the comparison group. Daptomycin did not prove non-inferior in the small subgroup with left-sided endocarditis. The label does not cover that use. 12
