Is Daptomycin safe?
Muscle and lung effects well documented
The main side effect is muscle damage. Creatine phosphokinase (CPK, a muscle enzyme) rises in a minority of patients. Muscle pain and weakness can follow, and rarely rhabdomyolysis. So the label calls for weekly CPK checks. People with kidney problems or on statins need checks more often. Eosinophilic pneumonia is a rare, known reaction. It is lung inflammation caused by the drug, and it usually shows up two to four weeks into treatment. Other common effects are diarrhoea, headache, insomnia and raised liver enzymes. 21
In the bacteremia trial, treatment failed in some patients and the bacteria became less susceptible to daptomycin. Most of these patients had deep infections that were not drained. Resistance is still rare, but doctors watch for it. 12
Muscle damage. Doctors check creatine phosphokinase weekly. They stop the drug if it rises sharply or muscle pain develops. Eosinophilic pneumonia is rarer but serious. 2
