Ecallantide

Does Ecallantide work?

Yes, in two controlled trials of 72 and 96 people.

AStrong human evidenceFDA approvedas of 2026-10-26

Approval was based on two randomized, double-blind, placebo-controlled trials, EDEMA3 (n=72, cited here) and EDEMA4 (n=96), both showing faster symptom relief than placebo during acute attacks. The trials were small by the standards of common diseases, but hereditary angioedema is rare and two consistent placebo-controlled trials meet the bar for A. 12

EDEMA3 randomized 72 patients having a moderate-to-severe attack. They got a single dose of ecallantide or placebo under the skin. At four hours, the median treatment outcome score was 50 on ecallantide and 0 on placebo. This score is a patient-rated measure of improvement. The median symptom severity score fell by 1.0 point versus 0.5. Both differences were statistically significant. The label reports the same trial as means of 63 versus 36 and 1.1 versus 0.6. 12

EDEMA4 was the second phase 3 trial. It enrolled 96 patients and found the same pattern. Symptom severity fell more at four hours (0.8 versus 0.4). The treatment outcome score was higher (53 versus 8). The FDA approved the drug on the two trials together. They covered 143 unique patients, since some took part in both. 2
