Plecanatide

Does Plecanatide work?

Yes, in large controlled trials, including one of 1,394 adults.

AStrong human evidenceFDA approvedas of 2026-10-26

Approval rested on randomized, placebo-controlled phase 3 trials; the published chronic constipation trial randomized 1,394 adults over 12 weeks, and companion trials covered IBS with constipation. The effect size is modest (roughly one in five responders versus one in ten on placebo), but the trial evidence for the approved use is as good as it gets. 12

A phase 3 trial studied chronic idiopathic constipation. It randomized 1,394 adults to one of two plecanatide doses or placebo for 12 weeks. The main measure was a durable overall responder. This meant at least three complete spontaneous bowel movements a week. It also meant a rise of at least one from baseline in most of the 12 weeks. About 20 percent of plecanatide patients met it (21.0 and 19.5 percent at the two doses). On placebo, 10.2 percent did. Stool consistency, straining and quality-of-life scores also improved. 1

Approval for IBS with constipation drew on two more 12-week phase 3 trials. In both, plecanatide improved the combined measure of belly pain and bowel movements more often than placebo. The prescribing information sums up these trials. 2
